Accident Book The accident book records all accidents or breakdowns that occur in the workplace. Even minor accidents such as scratches should be recorded. For each accident / breakdown, the following should be recorded: date, worker name, nature of the accident, cause of the accident, what measures should be taken to minimize the occurrence of the same accident and who should take such measures. Management, the health and safety committee and the workplace medical staff should use this record to track accidents / breakdowns and ensure that necessary measures are taken to reduce the number of accidents.
